Story:
K Muhammed (13) had applied to the City Corporation for his birth certificate. This is what he received: his ‘date of death’
courtesy a typo by a corporation clerk.

Story:
A sixth standard student in a school on the city’s outskirts, Muhammed had filled in the application form for the certificate earlier this month. He got the certificate on January 18. Though the first page mentions it as Birth Certificate, the second page where the details appear was full of mistakes. According to the document, the date of death of the boy was December 1, 1997 (which was actually his date of birth) and the place of death, a government hospital in the city. Even the date of grant of the certificate was wrongly mentioned as June 8, 2008.
